Key Responsibilities:
  • Conduct thorough assessments to identify patients' needs and develop personalized care plans
  • Manage patients with complex medical conditions, including acute medical situations and chronic conditions
  • Collaborate with interdisciplinary teams to ensure coordinated care and optimal patient outcomes
  • Provide advanced illness and end-of-life discussions, as well as counseling and education to patients and caregivers
Requirements:
  • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
  • Active and unrestricted license in the state where you reside, or ability to obtain by start date
  • Active Nurse Practitioner certification through a national board, such as the American Academy of Nurse Practitioners or the American Nurses Credentialing Center
  • Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations
Preferred Qualifications:
  • 1+ years of clinical experience in a long-term care setting
  • Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs
  • Understanding of Geriatrics and Chronic Illness
