Job Title: Clinical Nurse Specialist

We are seeking a highly skilled Clinical Nurse Specialist to join our team at Amergis. As a Clinical Nurse Specialist, you will work in a variety of settings, including clinics, wellness centers, and immunization sites, to provide high-quality patient care and promote health and wellness.

Responsibilities:
  • Evaluate patient health status and develop plans of care to address health risks and promote behavior change.
  • Provide education and support to patients and families to promote healthy behaviors and disease prevention.
  • Collaborate with healthcare teams to ensure seamless care coordination and communication.
  • Administer vaccines and medications as needed, and maintain accurate records of patient care.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements:
  • Current Registered Nurse License for the state in which the nurse practices.
  • One year experience as a nurse in a clinic or immunization setting preferred.
  • Compliance with all relevant professional standards of practice.
  • Participation and completion of Amergis' Competency program when applicable.
  • Current CPR certification if applicable.
  • TB questionnaire, PPD or chest x-ray if applicable.
  • Current Health certificate (per contract or state regulation).
  • Must meet all federal, state, and local requirements.
  • Successful completion of new hire training as applicable to job site.
  • Understand patient confidentiality and HIPAA requirements.
  • Ability to effectively communicate with patients, families, and healthcare teams.
