Job Summary

Key Responsibilities
  • Provide Skilled Nursing Care: Administer medications, perform wound care, and provide vital sign monitoring to patients in their homes.
  • Assess Patient Needs: Conduct thorough assessments to identify patient needs and develop personalized care plans.
  • Collaborate with Healthcare Team: Work closely with physicians, therapists, and other healthcare professionals to ensure seamless care coordination.
  • Communicate Effectively: Maintain open and honest communication with patients, families, and healthcare team members to ensure optimal care outcomes.
  • Document Patient Information: Accurately document patient information, including medical history, treatment plans, and progress notes.
Requirements
  • Current LPN License: Possess a current LPN license in good standing.
  • Experience in Home Health Care: Have at least 1 year of experience in home health care or a related setting.
  • CPR Certification: Hold current CPR certification from the American Heart Association or American Red Cross.
  • Strong Communication Skills: Possess excellent communication and interpersonal skills.
  • Reliable Transportation: Have reliable transportation to travel to patient homes.
