Nursing Leadership Home Health Coordinator

As a Patient Care Manager at Three Rivers Home Health Services, you will play a vital role in supervising and coordinating clinical services to provide quality care to patients in their homes. This position requires a Registered Nurse with home health experience, preferably.
Key Responsibilities- Supervise and coordinate clinical services to ensure patient needs are met.
- Provide nursing care to patients as prescribed by physicians and in compliance with applicable laws and regulations.
- Ensure clinician assignments are made appropriately and evaluate patient orders for start of care visits.
- Coordinate patient home health benefits, medical necessity, and ongoing insurance approvals.
- Current RN licensure in the state of practice.
- Current CPR certification required.
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
- A competitive estimated salary range of $75,000 - $95,000 per year, depending on experience.
