Job Summary and Qualifications

  • Currently licensed as an (RN) Registered Nurse in the state(s) of practice according to law and regulation

  • Associates degree required, BSN preferred

  • 2+ years experience in case management OR 3+ years experience in clinical nursing (i.e. acute care area: ICU, PCU, Med Surg- Tele, Oncology)

  • InterQual experience Preferred

  • Certification in Case Management preferred

About this Role

This is a critical role at HCA Florida Mercy Hospital where you will provide high-quality patient-centered care by managing rehabilitative care to ensure optimum outcomes. You will coordinate efforts within the rehabilitation team and collaborate with the rehabilitation program director and facility case management director regarding departmental functions.

Your Responsibilities

  • You will provide program orientation to patients/families/caregivers, which includes the case manager's role, rehabilitation philosophy, continued stay and discharge criteria, Medicare and insurance benefits, grievance procedures, treatment plan process, and rights and responsibilities

  • You will act as the coordinator of patient/family/caregiver education, promoting participation in team discussions related to plans, goals, and status through family conferences and other interactions

  • You will ensure implementation of the patient's treatment plan that supports the patient's strengths, abilities, needs, and preferences, facilitating involvement throughout the rehabilitation process

  • You will document findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments, communicating social, financial, or discharge needs and preferences of the patient/family/caregiver

  • You will assume accountability for promoting consistent, positive patient interactions that advance the agenda of unparalleled patient service

Salary Information
