Job Title: Licensed Clinical Social Worker

Job Summary:

The Licensed Clinical Social Worker will provide psycho-social support to Behavioral Health patients, facilitating coordination of social services to complement patient care and supporting the medical team in clarifying the goals of care and the plan of care.

Key Responsibilities:

  • Diagnose and treat common mental health problems and emotional difficulties, including anxiety disorders and depression, impact of trauma, medical illness or loss.
  • Provide individual therapy, group psychotherapy, psychoeducation, psychosocial assessment, and crisis intervention.
  • Collaborate with health professionals from other clinics.
  • Comply with clinic standards for timely submission, accuracy, and completeness of documentation in the EMR and collect all required statistical data.
  • Support and maintain a culture of safety and quality.
  • Attend department, hospital, and community in-services related to the field of practice.
  • Participate in quality improvement activities and program planning for the clinic, hospital, and service lines.
  • Assess and document patient and/or family psychosocial issues.
  • Treat patients and family with respect and dignity, maintaining appropriate patient satisfaction scores.
  • Work closely with all members of the healthcare team in developing the Plan of Treatment, including discharge plans, based on the psychosocial assessment and accepted social work interventions.
  • Develop and maintain thorough knowledge of referral resources throughout the service area and maintain up-to-date knowledge of Medicare, Medicaid, and other third-party reimbursement sources.
  • Provide positive customer relations in dealing with patients, families, physicians, members of the healthcare team, referral sources, service agencies, and others.
  • Adhere to all department/facility policies.
  • Behave ethically and professionally.

Requirements:

  • Master's degree in social work from a school accredited by the Council on Social Work Education.
  • Five years of full-time social work experience, including licensure as a clinical social worker by the California Board of Behavioral Sciences.
  • Three years' experience in all aspects of social services and discharge planning preferred.

  • Capable of working with diverse populations.
  • Emotional resilience and stability, as the work often involves dealing with crises.
  • Knowledge in caring for patients in different developmental stages throughout the life cycle.
