Job Summary

We are seeking a skilled Geriatric Care Coordinator to join our team at SSM Health. As a key member of our care team, you will be responsible for ensuring the highest level of care for our residents.

Key Responsibilities
  • Collaborate with the Interdisciplinary Team (IDT) to develop and implement individualized care plans that meet the unique needs of each resident.
  • Communicate effectively with residents, families, and healthcare providers to ensure seamless care transitions and coordination.
  • Monitor and update resident Minimum Data Set (MDS) to ensure accuracy and compliance with regulatory requirements.
  • Conduct regular care plan conferences with IDT members, residents, and families to review progress and make adjustments as needed.
  • Provide education and training to Certified Nursing Assistants (CNAs) and nursing staff on MDS-related requirements and best practices.
  • Obtain prior authorizations for new admissions as required and work with Admissions and Medical Records to ensure compliance with certification and re-certification regulations.
Requirements
  • Graduate of an accredited school of nursing or equivalent education for licensure.
  • Two years of geriatric experience with Medicare A coordination experience required.
  • Basic Life Support (BLS) certification and Registered Nurse (RN) licensure in the state of employment.
