As a Home Health Physical Therapist, you will play a vital role in promoting independence and enhancing the quality of life for our patients. Your primary responsibilities will include conducting in-home evaluations, developing individualized treatment plans, and implementing effective therapy techniques.

Key Responsibilities:

  • Conduct comprehensive in-home evaluations to determine patients' needs and develop tailored treatment plans
  • Implement prescribed physical therapy interventions using evidence-based techniques and modalities
  • Provide education to patients and their families on proper techniques and exercises for promoting mobility and functional independence
  • Monitor patient progress and revise treatment plans as necessary to achieve optimal outcomes
  • Document patient evaluations, treatment plans, and progress notes accurately and timely
  • Collaborate with interdisciplinary team members to ensure coordinated and comprehensive care

Requirements:

  • Valid and active Physical Therapist license in the state of Michigan
  • Prior experience as a PT in a home health care setting is preferred, but not required
  • Excellent communication and interpersonal skills
  • Ability to work independently and effectively manage time and caseload
  • Proficiency in electronic medical records and documentation
  • Compassionate and patient-centered approach to care
