Requirements:

We're looking for a Physical Therapist who is a graduate of a physical therapy curriculum approved by the American Physical Therapy Association and is licensed by the State of Michigan. One year of clinical experience is preferred. As a Physical Therapist on our Home Care team, you'll be responsible for providing physical therapy services to patients in their homes. This will include initial and ongoing assessments to determine the patient's level of functioning, problem identification and goal setting, developing the plan of treatment in consultation with the physician, and providing physical therapy in accordance with the plan of treatment. You'll also be responsible for revising the plan of treatment as needed and providing patient/family education as appropriate.
