Job Summary:We are looking for a compassionate and experienced Registered Nurse to provide high-quality patient care in a home health setting. As a Hospice RN, you will work closely with our interdisciplinary team to deliver individualized care to patients and their families.
Responsibilities:- Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
- Develop and implement individualized care plans in collaboration with the patient, family, and healthcare team.
- Conduct thorough assessments, identify patient needs, and prioritize care accordingly.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Participate in interdisciplinary team meetings to discuss patient progress and develop care plans.
- Current RN licensure in the state of Oregon.
- One to three years of experience as a Registered Nurse, preferably in a home health or hospice setting.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
- Ability to maintain confidentiality and handle sensitive patient information.
