POSITION OVERVIEW
The Licensed Practical Nurse (LPN) plays a vital role in delivering patient care through the nursing process. This includes data collection, assessment, and prioritization of patient needs, along with evaluating the effectiveness of nursing interventions.
The LPN is responsible for coordinating and directing patient education activities and ensuring seamless collaboration among health team members.
KEY RESPONSIBILITIES
In partnership with patients and healthcare teams, the LPN conducts comprehensive assessments to gather data tailored to individual patient needs.
In a timely and precise manner, the LPN collects both objective and subjective data, adjusting as necessary based on the patient's evolving condition, and communicates relevant findings to the healthcare team.
Adheres to all documentation standards within the scope of the LPN role.
Analyzes assessment data, evaluates patient conditions, and collaborates with healthcare providers to develop and maintain individualized care plans.
Provides education to patients, families, and staff in conjunction with other healthcare professionals, under the guidance of the provider or manager.
Coordinates patient care with other healthcare entities in alignment with the patient's care plan.
Administers medications safely, adhering to organizational policies and the State of Ohio Nurse Practice Act.
LICENSURE/CERTIFICATIONS
Current LPN licensure in the state of practice is required. IV certification may be necessary for specific specialties.
