Position Overview
We are seeking a dedicated and compassionate Registered Nurse Case Manager to deliver exceptional care to patients in their homes. This full-time role (32 hours per week) offers a $2500 Sign-on Bonus after six months of employment, with flexibility for those wishing to work up to 40 hours per week.
Key Responsibilities
- Conduct admission assessments, manage patient care, and perform follow-up visits utilizing the nursing process (assess, diagnose, plan, implement, evaluate).
- Collaborate with our interdisciplinary care team to develop and update individualized care plans for new patients.
- Provide comprehensive nursing services, including physical assessments, medication management, wound care, IV therapy, and patient education.
- Coordinate care with a variety of healthcare professionals, ensuring effective communication regarding patient needs and progress.
- Educate patients and their families on home health care practices to promote safety and well-being.
Qualifications
- Must possess a valid registered nurse license in New Hampshire or Maine.
- Strong organizational and communication skills are essential.
- Reliable transportation and a valid driver's license with auto insurance are required.
- Background checks will be conducted.
- Availability to rotate on weekends and holidays is preferred.
- Previous experience in home health care is advantageous, though training is available.
Compensation and Benefits
- Competitive pay at $34.00 per hour, including paid travel time and mileage.
