Job Title: Case Manager

We are seeking a skilled Case Manager to join our team at the Mental Health Association. As a Case Manager, you will play a critical role in providing support and guidance to individuals with severe and persistent mental illness.

Responsibilities:
  • Conduct intake assessments and develop treatment plans for individuals in the Assisted Outpatient Treatment (AOT) program.
  • Provide ongoing monitoring and support to ensure compliance with court orders and individual needs.
  • Collaborate with the treatment team to provide linkage and service coordination to appropriate treatment modalities.
  • Assist with obtaining public entitlements and benefits, such as Social Security, Medicaid, and Section 8.
  • Supervise and support recreational and socialization activities, as well as provide transportation and crisis intervention services as needed.
  • Meet productivity levels and document observations, information, and services in progress notes.
  • Participate in staff scheduling, training, and orientation.
  • Facilitate recovery groups and work with individuals on their Wellness Recovery Action Plan (WRAP).
Requirements:
  • Master's degree in social work, psychology, counseling, or a related field.
  • Demonstrated experience working with individuals with severe and persistent mental illness.
  • Valid driver's license in good standing.
