We are seeking a dedicated and compassionate Care Manager to join our team at Sunrise Senior Living. As a Care Manager, you will be responsible for providing exceptional care and services to our residents in our assisted living and reminiscence neighborhoods.
Key Responsibilities- Build Meaningful Relationships: Develop strong relationships with a specified number of seniors and their families, providing assistance with activities of daily living and attending to individual care needs.
- Notify Management of Changes: Inform management of any changes in a resident's condition and recommend adjustments in the level of care and service as needed.
- Assist with Life Skills: Assist residents with life skills and other life-enriching activities as indicated on their individual profile.
- Participate in Care Planning: Participate in the development of Individualized Service Plans (ISPs) and monthly updates.
- Ensure Resident Safety: Responsible for the safety and well-being of their designated group of residents during the shift, physically checking on them throughout the shift.
- Collaborate with the Community Team: Partner with the community team to ensure compliance with OSHA requirements and promote Risk Management programs and policies.
- Provide Excellent Customer Service: Respond to the dining needs of residents and guests while maintaining Sunrise hospitality and service standards.
- Dedication to Serving Seniors: A passion for serving seniors with excellent customer service skills.
- Positive Attitude and Flexibility: A positive attitude, flexibility to perform various duties, and the ability to work in a team environment are essential.
- Education and Certifications: A high school diploma or GED is required, and in some states, certifications may be necessary.
- Previous Experience: Previous experience working with seniors is preferred.
- Decision-Making and Communication Skills: Ability to make choices, decisions, and act in the resident's best interest, with strong written and verbal communication skills.
- Organizational and Time Management Skills: Competent in organizational and time management skills, with good judgment, problem-solving, and decision-making skills.
