Job Overview

The Role:

Saint Luke's Health System is in search of a Clinical Case Coordinator to enhance our dedicated team. This role is integral to the unit treatment team, collaborating with Behavioral Health Technicians, Registered Nurses, individual therapists, and medical professionals. Your skills and knowledge will be pivotal in providing support to the residents of our Psychiatric Residential Treatment Facility (PRTF) program, with a focus on delivering trauma-informed care to youth.

Work Schedule: Monday – Friday 8:30 AM - 5:00 PM

Key Responsibilities:

  • Organize follow-up appointments for residents post-discharge
  • Facilitate weekly treatment team meetings
  • Coordinate monthly treatment reviews
  • Execute case management tasks for residents, including managing clothing vouchers, visitations, school enrollments, etc.
  • Oversee referrals and admissions to our PRTF program

We are looking for a proficient case manager with experience in working with children. The ideal candidate will possess a strong understanding of Trauma-Informed Care principles. This position demands the ability to create effective communication systems to manage daily workloads efficiently. We seek an individual with excellent verbal and written communication skills, coupled with a compassionate and supportive demeanor. Success in this role requires strong collaboration skills, the ability to foster relationships, and the agility to adapt to changing priorities. This position necessitates working both as part of a team and independently.
