Case Manager I

Job Summary:
We are seeking a highly motivated and experienced Case Manager I to join our team at Tarzana Treatment Centers, Inc. As a Case Manager I, you will play a critical role in providing comprehensive case management services to our clients. Your primary responsibility will be to assess the needs of our clients and develop individualized treatment plans to address their unique challenges.
Key Responsibilities:
• Conduct comprehensive assessments of clients' needs and develop individualized treatment plans
• Provide case management services to clients, including connecting them with community resources and services
• Collaborate with multidisciplinary teams to ensure comprehensive care for clients
• Maintain accurate and up-to-date records of client progress and treatment plans
• Participate in regular team meetings to discuss client progress and treatment plans
Requirements:
• Bachelor's degree in a related field (e.g. social work, psychology, counseling)
• 1 year of experience in a behavioral healthcare setting
• Registration or certification with a DHCS-approved state-level counselor certification agency (for AOD programs)
• Bilingual English/Spanish preferred
• Valid California driver's license and insurance
