We are seeking a skilled Stabilization Case Manager to join our team at South Middlesex Opportunity Council. As a key member of our staff, you will provide critical support to families transitioning from emergency shelter into their own housing. Your expertise in case management, housing search, and community resource development will be instrumental in helping our clients achieve stability and self-sufficiency.
Key Responsibilities- Provide case management services to assigned clients, including initial and ongoing assessment, regular follow-up, and referrals to community-based services.
- Develop and maintain relationships with outside landlords and community providers to identify affordable housing options for families.
- Conduct home visits, provide coordinated housing search assistance, and support clients in establishing positive relationships with their landlords.
- Maintain accurate and up-to-date case notes, telephone contact logs, and referrals to community-based services.
- Work collaboratively with other SMOC staff on housing, employment search, and stability as indicated in the stabilization plan.
- Establish a respectful relationship with clients and schedule quarterly home visits to monitor the service plan.
- Support clients in their continued economic development planning and assist them in establishing sustainable housing alternatives.
- Bachelor's Degree preferred, or a minimum of three years' experience in Human Services or a related field.
- Minimum one year assessment, advocacy, and case management skills.
- Sensitivity to low-income people of diverse backgrounds.
- Excellent listening skills.
- Ability to work independently.
- Good written and verbal communication skills.
- Bi-lingual preferred (Spanish, Haitian Creole, and Portuguese are valued language skills).
- Must have a valid driver's license, reliable transportation, and meet insurance standards.
This position requires the ability to work in an office setting, with some travel via employee's personal car. Direct or incidental contact with clients served by SMOC in various programs funded or administered through the Executive Office of Health and Human Services is expected.
