Job Summary:

The Social Worker will work under the supervision of the Director for Social Work Services, Associate Director, or Social Worker Level V to provide biopsychosocial assessments, develop treatment plans, and provide ongoing supportive counseling to patients and their families. The ideal candidate will have a strong understanding of the landscape of homelessness in New York City and be able to navigate the complex systems of care.

Key Responsibilities:
  • Conduct biopsychosocial assessments and develop treatment plans for homeless patients
  • Provide ongoing supportive counseling to patients and their families
  • Collaborate with internal and external sources to provide comprehensive services to homeless patients
  • Coordinate with housing navigation teams to ensure patients receive necessary support
  • Complete referrals to crisis respite, medical respite, and other relevant services
  • Attend social work meetings and participate in supervisory conferences
  • Provide supervision and mentoring support to assigned social work staff
Requirements:
  • Valid license as an LCSW issued by the NYSED
