Job Summary

Rivers Health is seeking a per-diem Physical Therapist to perform the duties below:

Key Responsibilities
  • Evaluate and reassess patients referred to the department, planning and administering treatments and documenting patient's status and progress.
  • Maintain and upgrade evaluation and treatment skills.
  • Evaluate patient's problems subjectively and objectively and document data via SOAP format, set goals, plans, and implement treatment program.
  • Assess patient progress and alter treatment plan as needed.
  • Provide accurate and professional documentation as required by departmental policy.
  • Initiate physical therapy services upon physician referral and guidance of qualified physician.
  • Submit progress/discharge reports to physicians and other referral sources on a regular basis.
  • Provide patient care utilizing physical agents and therapeutic exercise upon referral and guidance of qualified physician.
  • Supervise P.T.A.'s, rehab techs, students, and volunteers.
  • Assist with orientation and training of new personnel.
  • Interact with other team members to provide total care of patients.
  • Participate in staff meetings and in-services.
  • Keep up to date in the profession through literature, workshops, seminars, etc.
  • Responsible for professional growth.
  • May function as clinical instructor for physical therapy students.
  • Demonstrate excellence in customer service skills.
  • Foster an atmosphere of teamwork and collaboration.
  • Participate in quality improvement activities including serving on quality improvement teams.
  • Maintain cardiopulmonary resuscitation certification.
Qualifications
  • Four or five years of formal education and Bachelor/Master of Science degree or equivalent in Physical Therapy from an accredited institution.
  • Current West Virginia Physical Therapy License.
