About the Role

We are seeking a skilled and compassionate LPN to join our team as a Home Health Nursing Professional. As a key member of our care team, you will provide high-quality nursing care to patients in their homes, promoting independence, dignity, and comfort.

Responsibilities
  • Provide professional nursing services under the direction of a RN, adhering to state and federal regulations, and CenterWell Home Health policies and procedures.
  • Develop and implement individualized care plans, focusing on patient-centered care and outcomes.
  • Confer with RN Supervisors to ensure seamless care coordination and communication.
  • Maintain accurate and timely documentation, adhering to company standards and regulatory requirements.
  • Collaborate with interdisciplinary teams to ensure comprehensive patient care and support.
Requirements
  • Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
  • Current nursing license in the practicing state.
  • Valid drivers license, auto insurance, and reliable transportation.
  • Current CPR certification.
  • Two years experience as an LPN/LVN in a clinical setting, preferably in home health or hospice.
