Overview

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Team Select Home Care. As a home health nurse, you will have the opportunity to provide high-quality patient care in the comfort of their own homes.

Responsibilities
  • Provide 1:1 patient care to pediatric and adult patients with g-tube, trach, and/or ventilator
  • Document electronically and maintain accurate patient records
  • Conduct patient assessments and develop meaningful long-term relationships
  • Administer medications and chart patient progress
  • Communicate with additional care providers and families
