Nursing Care Overview:
The Nursing Care Coordinator will be responsible for administering nursing care to patients in their place of residence, coordinating care with interdisciplinary teams, and assessing patient needs to develop individualized plans of care.
Key Responsibilities:
- Assess patient needs, develop individualized plans of care, and implement revisions as necessary.
- Provide clinical directions to Hospice Aides and LPNs/LVNs, ensuring quality and continuity of service.
- Consult with patients and families regarding disease processes, self-care techniques, end-of-life care, nutrition, and dietary needs.
- Maintain compliance with third-party payor requirements and demonstrate commitment to professional growth and competency.
Requirements:
- Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
- Current license as a registered nurse (RN) and/or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC).
- Two years of experience as a Registered Nurse in a clinical care setting, preferably hospice.
- Nursing skills as defined by generally accepted standards of practice.
Salary:$93,000 per year, depending on experience.
