About the Role

We are seeking a skilled Licensed Vocational Nurse / Licensed Practical Nurse (LVN/LPN) to join our team at Santa Clara Post Acute. As a key member of our healthcare team, you will provide high-quality care to our residents, ensuring their physical and emotional needs are met.

Responsibilities
  • Deliver a full range of nursing care, including medication administration, monitoring resident care, and maintaining confidentiality of resident care information.
  • Conduct daily rounds to observe residents' physical and emotional status, and provide individualized care as needed.
  • Work collaboratively with the healthcare team to ensure seamless care delivery and promote a culture of safety.
Requirements
