Job Title: Health Home Care Coordinator

We are seeking a highly skilled and compassionate Health Home Care Coordinator to join our team at Jewish Family Services of Western New York. As a key member of our care coordination division, you will work closely with clients, healthcare providers, and community resources to ensure seamless care coordination and support.

Job Summary:

The Health Home Care Coordinator will be responsible for providing care coordination services to clients enrolled in the Health Home program, including those with Serious Mental Illness (SMI) and Substance Use Disorder (SUD). You will conduct comprehensive assessments, develop and maintain individualized care plans, and link clients to community-based services and resources. Your expertise will help clients achieve their goals and improve their overall well-being.

Key Responsibilities:
  • Conduct home visits and community-based assessments to identify client needs and develop care plans.
  • Collaborate with healthcare providers, community resources, and family members to ensure comprehensive care coordination.
  • Link clients to community-based services, including Home and Community-Based Services (HCBS) and Community Oriented Recovery and Empowerment Services (CORE).
  • Monitor client progress, make adjustments to care plans as needed, and ensure timely follow-up with clients and healthcare providers.
  • Provide education and support to clients and their families on health home benefits, HCBS, and CORE services.
  • Participate in interdisciplinary care team meetings to share client information and coordinate care.
  • Utilize electronic systems, including Netsmart, HEALTHeLink, PSYCKES, and ePACES, to document client interactions and track progress.
Requirements:
  • Bachelor's degree in a related field, such as social work, counseling, or healthcare administration.
  • Two years of experience providing direct services to individuals with SMI, SUD, or other behavioral health diagnoses.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to work independently and as part of a team, with a focus on client-centered care.
  • Valid driver's license and reliable vehicle for travel between sites.
