Under the supervision of the Administrator-LTC, Assistant Administrator, or Assistant Director of Nursing, this Social Worker will provide social work services and discharge planning services to residents and their families.

Responsibilities:
  • Provide social work services to residents and their families
  • Develop and implement discharge plans
  • Collaborate with healthcare teams to ensure resident-centered care
  • Conduct assessments and develop care plans
  • Provide education and support to residents and families
Requirements:
  • BS Degree in Social Work or related field
  • New York Screen certified
  • One year direct service experience in a health or long-term care setting
  • Excellent interpersonal, communication, and organization skills
  • Computer literacy
Preferred Qualifications:
  • Long-term care experience
  • Experience with Microsoft Office products
Salary Range:

$26.00-$28.72
