Clinical Nurse Specialist

Vivo HealthStaff is seeking a skilled and compassionate LVN to join our team in providing high-quality care to our geriatric patients. As a Clinical Nurse Specialist, you will be responsible for coordinating and providing direct and indirect nursing care to patients in their homes and at our clinic.
Key Responsibilities:
- Work under the supervision of the Clinic Director/Registered Nurse and maintain vocational nursing professional standards of care.
- Perform home visits to administer prescribed medications and treatments in accordance with nursing standards to supplement clinic services.
- Observe, record, and report patients' condition and reaction to medication and treatments to physicians.
- Document all interactions related to clinic visits in the electronic medical record.
- Instruct patients and family members regarding medications and treatment instructions in the home by CNAs, PCWs, and homemakers.
- Keep Home Care Supervisor and Interdisciplinary team informed on home care issues of patients.
- Attend morning meetings at the center, family meetings, and any other meetings as assigned.
- Complete all required documentation in a timely and accurate manner.
- Protect privacy and maintain confidentiality of all company procedures and information about employees, patients, and families.
- Follow policies and procedures and participate in any required Quality Improvement activities, staff training, and meetings.
- Perform other duties as assigned.
- Requires the use of a personal vehicle.
Requirements:
- Graduate of an accredited school of nursing.
- Unencumbered California Licensed Vocational Nurse (LVN) License.
- Current Basic Life Support (BLS) / First Aid Certification required.
Preferred Qualifications:
- Minimum of two years nursing experience in a clinical setting with a frail or elderly population.
- Nursing knowledge and skills necessary to treat frail, elderly patients and manage complex clinical situations.
- Highly motivated, self-directed, able to execute tasks in a quickly changing environment and make sound decisions in emergency situations.
- Excellent clinical, organizational, and communication skills in settings with seniors, their families, and interdisciplinary team members.
- Experience and competency working with people from diverse backgrounds.
- Commitment to unlocking the full potential of our most vulnerable seniors.
- Bilingual Spanish-English preferred.
