Key Responsibilities
  • Collaborate with Care Team: Work closely with a multidisciplinary care team to identify patient treatment goals and barriers to care.
  • Patient Education: Educate patients and their families on Patient Centered Medical Home (PCMH) concepts and promote adherence to care plans.
  • Resource Connection: Connect patients to relevant community resources to enhance patient health and well-being.
  • Referral Processing: Process referrals for high-risk populations and ensure follow-up as needed.
  • Data Collection: Document pertinent care management information and assist with data collection and generation of patient reports.
  • Quality Improvement: Participate in quality improvement activities and attend in-person case conferences and periodic meetings.
Requirements
  • Education: High school diploma or equivalent.
  • Experience: Graduate of an accredited Medical Assistant Program or at least one year of care coordination experience.
  • Language: Bilingual in English and Spanish.
  • Skills: Knowledge of medical terminology, care coordination, and computer proficiency.
Preferred Qualifications
  • Education: Associate's degree in social work, human services, counseling, or related field.
  • Experience: Two years of experience in chronic disease management, case management, utilization management, and acute care.
