A-Line Staffing Solutions is seeking a highly motivated and compassionate Certified Community Health Worker to join our team in the Orlando, FL area. As a key member of our team, you will be responsible for conducting member-facing visits, providing community engagement, care planning, and assessment completion.

**Key Responsibilities:**

* Conduct field work for member outreach and community engagement
* Utilize clinical documentation system for outcome reporting
* Serve as the health plan liaison for new/existing community based organizations
* Complete health risk assessments/screenings and address social barriers
* Meet performance expectations, including member and/or provider outreach compliance and quality case review audits

**Requirements:**

* Community health worker CHW Certification
* Relevant case management, community engagement, care planning, assessment completion
* Valid driver's license and reliable transportation
* High-speed wired internet and quiet/secure home work space
* Working knowledge of Social Determinants of Health (SDOH) barriers and geographic area
* Working knowledge of motivational interviewing and care plan development/goal setting
* High school diploma or GED and general understanding of Microsoft office suite
