Acute Care Nurse Practitioner or Physician Assistant

Key Responsibilities
- Conducts thorough evaluations of patients to determine immediate care requirements. Records patient assessments and identifies educational needs. Gathers comprehensive medical histories and recognizes patients at both physiological and psychological risk. Collaborates with the interdisciplinary team regarding assessment results.
- Executes necessary procedures to carry out the care plan in accordance with established standards and protocols.
- Formulates a comprehensive care plan and utilizes discretion and judgment to modify it based on physiological and psychosocial insights. Orders necessary tests and consultations as dictated by the care plan and reviews the results. Engages with the interdisciplinary team to discuss the patient’s care plan and communicates it effectively to patients and their families.
- Maintains accurate documentation of all patient care activities in line with documentation standards. Assesses the effectiveness of documentation and contributes to quality improvement initiatives through medical record evaluations.
- Acts as a catalyst for change by implementing innovative practice initiatives. Coordinates patient care activities to enhance patient outcomes.
- Facilitates the evaluation of patients' educational needs and measures the outcomes of educational interventions, leveraging resources from various disciplines to achieve optimal patient outcomes.
- Enhances communication across departments to ensure seamless continuity of care. Serves as a primary resource for the patient care team. Identifies, develops, and maintains educational resources for the patient care team.
- Leads quality improvement initiatives. Identifies trends within patient populations and explores alternative care options while assisting in the revision of standards, guidelines, and protocols. Analyzes practices contributing to variances, draws conclusions from aggregated data, and identifies resources to aid in variance analysis.
- Contributes to the development and revision of standards for patient populations by incorporating an interdisciplinary approach and utilizing evidence-based practices and research findings. Designs and updates patient education programs and assesses their effectiveness. Engages in clinical research and reviews current nursing literature, demonstrating its application in clinical practice.
- Recognizes behaviors that influence team dynamics and implements strategies to enhance teamwork, serving as a mentor for colleagues. Pursues leadership opportunities and fosters leadership development among peers. Collaborates in creating and executing educational plans for identified needs.
- Reviews personal development plans and professional objectives, seeking new knowledge to advance career goals. Demonstrates commitment to self-improvement by participating in educational programs and providing training to elevate staff performance.
Qualifications
For Physician Assistant:
- Graduation from an accredited physician assistant program recognized by the National Commission of Certification of Physician Assistants (NCCPA).
- Preferred one year of experience in primary care management or acute care settings.
- Possession of a valid NJ Physician Assistant License.
For Nurse Practitioner:
- Bachelor's degree in nursing and a Master's degree in advanced clinical practice or completion of a post-Master's certificate program for nurse practitioners.
- Preferred one year of experience in primary care management or acute care settings.
- Possession of an Advanced Practice Nurse - NJ License.
