About the Role

We are looking for an experienced nurse who can navigate and coordinate patient care across the continuum. The ideal candidate will have a strong understanding of health promotion and clinical outcomes, as well as excellent communication skills.

Responsibilities
  • Assess, plan, and evaluate patient care plans in collaboration with the healthcare team.
  • Monitor patient care processes and collect data to improve quality outcomes.
  • Collaborate with physicians and other healthcare professionals to provide comprehensive care.
Requirements
  • Associate's degree in Applied Science in Nursing or higher.
  • Minimum 2 years of experience in acute/tertiary facilities.
