Job Summary:The Home Care Physical Therapist Specialist will be responsible for assessing patients' physical needs, developing personalized treatment plans, and implementing evidence-based interventions to enhance functional mobility, reduce pain, and improve overall well-being.
Key Responsibilities:- Evaluate patients' physical condition, identifying areas of impairment and developing targeted treatment plans to address these issues.
- Conduct thorough assessments of patients' strength, flexibility, balance, and range of motion, utilizing standardized evaluation tools and techniques.
- Design and implement individualized exercise programs, incorporating modalities such as therapeutic exercises, manual therapy, and education on proper body mechanics and postural awareness.
- Collaborate with other healthcare professionals, including physicians, occupational therapists, and speech-language pathologists, to ensure comprehensive care and promote effective communication.
- Document patient progress, including assessment results, treatment plans, and outcome measures, to maintain accurate records and facilitate continuity of care.
- Bachelor's degree in Physical Therapy from an accredited program.
- Current licensure as a Registered Physical Therapist in the State of Connecticut.
- AHA CPR certification.
- Excellent written and verbal communication skills, with ability to effectively communicate with patients, families, and healthcare teams.
- Strong analytical and problem-solving skills, with ability to adapt to changing situations and prioritize tasks accordingly.
We offer a competitive salary range of $95,000 - $135,000 per year, commensurate with experience, plus a comprehensive benefits package, including medical, dental, vision, life insurance, and retirement plans.
