About the Role

We are seeking a skilled Nurse Care Coordinator to join our team at ResCare Community Living. As a key member of our healthcare team, you will play a vital role in providing high-quality patient care and support to individuals with intellectual, developmental, or cognitive disabilities.

Key Responsibilities
  • Provide 1:1 care for clients, following their individualized care plans and promoting their dignity and independence.
  • Monitor clients' vital signs and report any abnormal findings to the case manager and physician.
  • Administer medications, feedings, oxygen, and other treatments as needed, and document the results.
  • Collaborate with other healthcare professionals, such as physical therapists, occupational therapists, and speech therapists, to ensure comprehensive care.
  • Ensure infection control policies are maintained at all times and report any potential communicable diseases or infections.
  • Accurately document nursing actions and communicate with family members, case managers, and physicians to ensure seamless care coordination.
Requirements
  • Completed post-secondary coursework to facilitate licensure as a Licensed Practical Nurse or Licensed Vocational Nurse.
  • Knowledge of individual service plans and understanding of client rights and health needs.
  • Proof of current license as a Licensed Practical Nurse or Licensed Vocational Nurse in good standing from the state(s) of practice.
  • CPR certification.
  • Excellent interpersonal skills, including customer service, patience, empathy, and compassion.

Salary and Benefits

Salary range: USD $28.00/Hr. - USD $31.00/Hr.
