Job Summary

Prairie Lakes Health Campus is seeking a highly skilled and experienced Director of Social Services to lead our Social Services Department. As a key member of our team, you will be responsible for directing, planning, organizing, and developing the operation of the Social Services Department.

Key Responsibilities
  • Direct and manage the Social Services Department, ensuring compliance with federal, state, and local standards, guidelines, and regulations.
  • Participate in the implementation of social care plans and resident assessments, ensuring that the medically related emotional and social needs of residents are met and maintained.
  • Develop and implement social service programs, including discharge planning, social care plans, and resident assessments.
  • Review and revise care plans and assessments as necessary, ensuring that they are up-to-date and meet the needs of residents.
  • Provide information to residents and their families on Medicare, Medicaid, and other financial assistance programs.
  • Develop and implement plans of correction for social service deficiencies noted during survey inspections.
  • Interview residents and their families to obtain social history and provide support as needed.
  • Attend and participate in annual company-required in-service training programs.
  • Follow all safety regulations, programs, policies, and procedures.
  • Follow resident rights and responsibilities, including the right of refusal.
  • Follow company policies and procedures regarding resident complaints and grievances.
  • Assist in recruiting, screening, selecting, hiring, training, coaching, counseling, and developing a team of qualified and highly motivated individuals.
Requirements
  • High school diploma or equivalent required; Bachelor's degree in Psychology or related field preferred.
  • Three to five years' experience working in the field of social services in a senior residential care, healthcare, senior living industry, or long-term care environment.
  • Exemplary computer skills, including knowledge of the Microsoft Office Suite of products.
  • Must meet state requirements and regulatory code(s) for licensure or certification.
Physical Requirements

Sitting, standing, bending, reaching, stretching, stooping, walking, and moving intermittently during working hours. Must be able to lift at least 50 pounds. Must be able to maintain verbal and written communication with co-workers, supervisors, residents, family members, visitors, vendors, and all business associates.
