We are seeking a highly skilled and compassionate Social Worker to join our team at the Indiana Hemophilia & Thrombosis Center, Inc. As a Social Worker, you will play a vital role in protecting and improving the social and mental well-being of our patients and their families.

Key Responsibilities
  • Participate in comprehensive clinics to assess the psychosocial needs of patients and families.
  • Collaborate with multidisciplinary team members to establish an appropriate plan of care.
  • Consult with healthcare providers to provide psychosocial services as needed.
  • Educate patients and families about available community resources.
  • Act as an advocate, coordinating and connecting patients with services and assistance programs.
  • Provide therapeutic psychosocial assessment, supportive counseling, crisis intervention, and monitoring.
  • Contribute to discharge planning for hospitalized patients.
  • Participate in events, education programs, and other initiatives as requested.
  • Monitor legislation impacting individuals with chronic disorders.
  • Communicate with outside agencies and organizations to promote our services.
  • Coordinate with billing and insurance departments to facilitate resolution of related issues.
Requirements
  • Minimum 3 years of experience in a healthcare setting.
  • Master's degree in Social Work required.
  • Valid Licensed Clinical Social Worker (LCSW) in the state of Indiana strongly preferred.
  • Valid driver's license and automobile insurance required.
