As a Private Duty Nurse LPN, you will be responsible for implementing the physician's prescribed plan of care, conducting patient assessments, and coordinating care with the healthcare team.
Responsibilities- Implement the physician's prescribed plan of care, ensuring the patient's needs are met and their condition is stable.
- Conduct regular patient assessments to identify any changes in the patient's condition and adjust the care plan accordingly.
- Coordinate care with the healthcare team, including physicians, therapists, and other healthcare professionals.
- Administer prescribed medication, treatments, and therapies as ordered by the physician.
- Document all care provided in a timely and accurate manner, ensuring compliance with regulatory requirements.
- Provide health education and training to family members, empowering them to provide ongoing care and support.
- Communicate effectively with the healthcare team, family members, and other stakeholders to ensure seamless care coordination.
- Active, unencumbered LPN license in the state where you will practice.
- Current CPR certification from a recognized organization.
- TB skin test (current within the last 12 months).
- At least six months of prior hands-on nursing experience, preferably in a pediatric or high-acuity setting.
- Reliable transportation to and from patient homes.
