Key Responsibilities
  • Implement Care Plans: Develop and implement individualized care plans in collaboration with the patient's healthcare team.
  • Patient Assessments: Conduct thorough assessments of the patient's physical and emotional needs, identifying areas for improvement and developing strategies for care.
  • Medication Administration: Administer prescribed medications, treatments, and therapies as ordered by the patient's physician.
  • Documentation: Maintain accurate and timely documentation of patient care, including progress notes, medication administration records, and other relevant information.
  • Health Education: Educate patients and their families on healthy habits, disease management, and other relevant topics to promote overall well-being.
  • Communication: Maintain open and effective communication with the patient's healthcare team, including physicians, therapists, and other caregivers.
Requirements
  • Licensure: Must have and maintain an active, unencumbered license as a Registered Nurse (RN), Licensed Practical Nurse (LPN), or Licensed Vocational Nurse (LVN) in the state where you will be practicing.
  • CPR Certification: Current certification in cardiopulmonary resuscitation (CPR) from the American Red Cross or American Heart Association.
  • TB Skin Test: Must have a current TB skin test (within the last 12 months).
  • Experience: Six months of prior hands-on nursing experience, preferably in a pediatric or home health setting.
  • Transportation: Must have reliable transportation to and from patient locations.
