Patient Care Specialist

As a Patient Care Associate at Northwell Health, you will play a vital role in providing direct care to patients in a designated group. Your responsibilities will include assisting with admission and discharge processes, establishing good interpersonal relationships with families and hospital staff, and reporting vital information to the healthcare team. You will also be responsible for conveying needed information to patients and visitors, maintaining good communication with co-workers, and performing direct patient care as assigned. Additionally, you will obtain and document patient vital signs, apprise the Registered Nurse and healthcare team of any observed significant changes in the patient's condition, and respond to patient requests and/or call light. You will maintain a safe, comfortable, and therapeutic environment for patients and families in accordance with hospital standards. You will also function in an emergency situation as a team member with guidance from supervisory personnel. You will assist with restrained patients according to established policy and perform grade I and II decubitus care, as directed. You will also perform activities of daily living appropriate to the patient's condition, use proper body mechanics, and obtain help and/or use appropriate equipment when lifting, moving, or transferring patients. You will perform delegated patient care procedures, obtain and label specimens, and perform certain diagnostic tests consistent with established procedures, as directed. You will also perform phlebotomy procedures, telemetry, and electrocardiograms, as directed. You will use clinical knowledge in the care and management of patients based on diagnoses and the unit assigned. You will perform related duties as required. Qualifications
To be successful in this role, you will need a High School Diploma or equivalent, proficiency in performing EKG and Phlebotomy, and a minimum of one year of direct patient care experience. You will also need to be able to establish good interpersonal relationships with families and hospital staff, communicate effectively with the healthcare team, and maintain a safe and therapeutic environment for patients and families.
