**Job Summary:**

We are seeking a Healthcare Support Professional to join our team at St. John's Community Services in Washington, D.C. This role involves providing coordinated healthcare services to individuals with intellectual and developmental disabilities (IDD).

**Key Responsibilities:**

  • Coordinating medical appointments and communicating effectively with healthcare professionals.
  • Maintaining accurate and timely documentation of appointment details, diagnoses, treatment plans, and follow-up recommendations.
  • Collaborating with the support team to ensure continuity of care and implementing prescribed treatments, dietary restrictions, and medical equipment needs.

**Requirements:**

  • A minimum of 1 year of experience working with individuals with IDD or in a healthcare setting.
  • Strong verbal and written communication skills, with the ability to interact effectively with healthcare professionals and support teams.
  • Proficiency in using Therap or similar documentation systems.

**Compensation and Benefits:**

  • A competitive hourly rate of $18.00.
  • A comprehensive benefits package, including medical, dental, vision, STD, and other benefits and company contributions.
