Nurse Practitioner Specialist

As a skilled nursing facility's leading provider of internal medicine services, you will collaborate with the facility's interdisciplinary team and physicians to serve as a day-to-day clinical leader. Your role will involve overseeing the care of all residents, working with the entire patient population, and providing comprehensive health assessments, differential diagnosis, and prescribing treatments under the protocol in the direct management of acute and chronic illness and disease.
Key Responsibilities:- Evaluation of new-admission visits
- Coordination of discharge visits
- Monthly stability visits for long-term residents
- Evaluations for patients exhibiting changes in condition
- Prescribing all necessary medications and treatments
- Performing comprehensive physical examinations of patients
- Developing and implementing patient management policies and procedures
- Facilitating referrals to other healthcare professionals and medical facilities
- Communicating with attending physicians and/or specialists regarding patient care

- Salary Ranges (Commensurate with experience): $105,000 - $125,000+

- Previous experience in nursing or other medical fields
- Ability to build rapport with patients and their loved ones
- Ability to thrive in a fast-paced environment
- Excellent written and verbal communication skills
- Strong leadership experience with emphasis on teamwork in a healthcare setting
- Master's degree required
