Job Title: Nurse Practitioner

Monogram Health is seeking a skilled Nurse Practitioner to join our team of healthcare professionals. As a key member of our integrated Care Team, you will play a vital role in delivering high-quality, patient-centered care to individuals with chronic kidney disease and related metabolic disorders.

Job Summary:

We are looking for a compassionate and experienced Nurse Practitioner to provide comprehensive care to our patients in the comfort of their own homes. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Responsibilities:
  • Conduct comprehensive annual wellness exams and provide personalized care to patients with chronic kidney disease and related metabolic disorders.
  • Counsel and educate patients and families about benefits and programs available to help them live healthier lives.
  • Document patient information, including medical history, medications, and treatment plans.
  • Collaborate with primary care providers, specialists, and ancillary services to ensure seamless care coordination.
  • Prescribe medications, order tests, and work with patients' Monogram physicians to develop effective treatment plans.
  • Participate in integrated care team meetings and contribute to the development of patient care plans.
Requirements:
  • Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license.
  • Board certified for appropriate licensure (NP: ANCC/AANP; PA: NCCPA).
  • Current and unrestricted DEA certificate.
  • Ability to work autonomously and practice without direct supervision.
  • Access to transportation, a valid driver's license, and car insurance.
  • Proficiency with medical instruments and equipment required by the work.
  • Knowledge of computer-based data management programs and information systems.
  • Ability to communicate effectively with patients, families, physicians, and community representatives.
  • Sound understanding of federal and state regulations, including HIPAA and OSHA.
  • 2 or more years of direct patient care experience.
  • Managed Care/IPA/Health Plan experience.
