Job Summary:

This is a temp-hire position starting in October 2024, with a competitive hourly pay range of $62.50 to $76.92 per hour. The successful candidate will be responsible for providing comprehensive primary health care services to clients, coordinating patient care with other providers and agencies, and advocating for clients within healthcare systems.

Key Responsibilities:
  • Provide comprehensive primary health care services to clients.
  • Coordinate patient care with other providers and agencies.
  • Advocate for clients within healthcare systems.
  • Perform initial and ongoing health risk assessments and counseling for high-risk lifestyle behaviors and/or familial diseases.
  • Develop and document plans of care and outcomes evaluations.
  • Coordinate with contractual pharmacy, primary care providers, hospitals, and laboratories for required diagnostic procedures and delivery/births.
  • Provide health education to individual patients as needed.
  • Participate in ongoing education and specialized training to maintain a high quality of care to the Clinic's patient population.
Qualifications:
  • Evidence of experience sufficient for successful performance of duties.
  • Ability to maintain professional boundaries with clients, the community, and other staff members.
  • Work collaboratively with clients, other staff, team members, and treat everyone with respect and dignity at all times.
