Family Medicine Physician

UnityPoint Health is seeking a highly skilled Family Medicine Advanced Practitioner to join our team in Deforest, WI. As a member of our UnityPoint Clinic team, you will have the opportunity to provide high-quality patient care and contribute to the health and well-being of our community.
Responsibilities- Provide comprehensive family medicine services, including routine check-ups, health screenings, and treatment of acute and chronic conditions.
- Collaborate with other healthcare professionals to ensure seamless care coordination and patient outcomes.
- Develop and implement individualized treatment plans, taking into account patient preferences and health goals.
- Stay up-to-date with the latest medical research and best practices in family medicine.
- Nurse Practitioner or Physician Assistant certification.
- Board certification in Family Medicine.
- Minimum 2 years of experience in family medicine, preferably in an outpatient setting.
- Unrestricted medical license in the state of Wisconsin.
- Current and unrestricted state and federal authority to prescribe and administer medications.
