Patient Care Manager RN

We are seeking a skilled and compassionate Patient Care Manager RN to join our team at Caretenders, a part of LHC Group. As a Patient Care Manager RN, you will play a vital role in providing quality support to our patient-facing teams and ensuring the best possible outcomes for our patients.
Key Responsibilities:
- Supervise and coordinate clinical services to ensure high-quality patient care.
- Provide and direct provisions of nursing care to patients in their homes as prescribed by the physician.
- Ensure patient needs are continually assessed and care rendered is individualized to patient needs.
- Coordinate determination of patient home health benefits, medical necessity, and ongoing insurance approvals.
Requirements:
- Current RN licensure in the state of practice.
- Current CPR certification.
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
