Job Title: Patient Care Manager RN

Job Summary:

We are seeking a highly skilled and compassionate Patient Care Manager RN to join our team at LHC Group. As a Patient Care Manager RN, you will play a vital role in ensuring the delivery of high-quality, patient-centered care in the comfort of patients' homes.

Responsibilities:

* Provide clinical leadership and supervision to a team of healthcare professionals, ensuring that patients receive the best possible care.

* Develop and implement individualized care plans that meet the unique needs of each patient.

* Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and communication.

* Conduct regular assessments and evaluations to ensure that patients are receiving the necessary care and services.

* Identify and address any potential issues or concerns that may impact patient care.

* Develop and maintain relationships with patients, families, and healthcare providers to ensure a high level of satisfaction and trust.

* Stay up-to-date with the latest clinical guidelines, regulations, and best practices to ensure that patients receive the most effective and efficient care.

* Participate in quality improvement initiatives to continuously improve patient care and outcomes.

Requirements:

* Current RN licensure in the state of practice.

* Current CPR certification.

* Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
