Responsibilities:

  • Conduct initial comprehensive assessments of patients to determine hospice needs, focusing on pain and symptom management.
  • Ongoing assessments of patients' physical status and psychosocial well-being.
  • Develop and implement individualized plans of care in collaboration with the IDG, patients, and physicians.

Requirements:

  • Graduate of an accredited RN program.
  • Active, unrestricted RN license in South Carolina or compact multi-state license.
  • Twelve months of accumulated RN experience within the last five years.
  • One year of hospice care experience preferred.
