Palliative Medicine Nurse Practitioner Position:

We are seeking a skilled Nurse Practitioner to join our team at Greenstaff Physician & Provider Solutions in Hancock County, OH. As a key member of our healthcare team, you will provide high-quality care to patients and their families, focusing on symptom management, chronic disease management, and advanced care planning.

About the Role:

  • Provide palliative care services to patients with complex medical needs
  • Work collaboratively with hospitalists and other healthcare professionals to deliver comprehensive care
  • Develop and implement individualized treatment plans to address patient symptoms and improve quality of life
  • Conduct regular assessments and evaluations to ensure optimal patient outcomes

Job Requirements:

  • Board certification as a Nurse Practitioner
  • Geriatric certification or willingness to obtain within 1 year
  • Ohio license or ability to obtain
  • Minimum 3 years of experience as a Nurse Practitioner in an acute care setting or with palliative care and hospice experience
