Responsibilities
  • Lead a team of 4 direct reports, including 2 full-time Case Managers, 1 part-time Case Manager, and 1 Case Management Assistant.
  • Collaborate with the senior leadership team to develop and implement case management strategies.
  • Ensure compliance with regulatory requirements and industry standards.
  • Develop and maintain relationships with healthcare providers and payers.
  • Stay up-to-date with industry trends and best practices in case management.
Requirements
  • Licensed RN with a BSN or a Licensed Social Worker degree.
  • Current CCM or ACM certification required or must be obtained within one year.
  • 3+ years of hospital-based Case Management experience, including Utilization Review and Discharge Planning.
  • At least 1 year of case management supervision experience preferred.
