Job Overview

At Phoenix Home Care & Hospice, we're committed to providing exceptional care to our clients. As a Pediatric Private Duty LPN Nurse, you'll play a vital role in delivering high-quality care to children and their families.

Key Responsibilities:

  • Develop, review, and revise individual plans of care in collaboration with the healthcare team.
  • Provide nursing care in accordance with physician orders and client payer sources.
  • Monitor and record patient reactions to treatment and changes in condition.
  • Supervise and instruct clients, families, or other healthcare team members on plan of treatment.
  • Document patient care services via electronic charting.

Requirements:

  • Active Licensed Practical Nurse or Registered Nurse License.
  • Problem-solving skills and ability to multitask.
  • Excellent communication skills.
  • Positive, professional attitude.
