We are seeking a highly skilled Care Manager II, Registered Nurse to join our team at Sutter Health. As a key member of our care coordination team, you will play a critical role in ensuring the timely progression and transition of patients to the appropriate level of care, preventing unnecessary admissions or readmissions.
Key Responsibilities- Collaborate with physicians, utilization managers, medical social workers, and bedside RNs to coordinate care and facilitate transitions.
- Develop and implement effective care plans that address the unique needs of each patient.
- Communicate with patients, families, and caregivers to ensure seamless transitions and optimal health outcomes.
- Work closely with the interdisciplinary team to ensure timely and integrated care coordination.
- Monitor and analyze data to identify trends and opportunities for improvement in care coordination and patient outcomes.
- Graduate of an accredited school of nursing.
- RN-Registered Nurse of California upon hire.
- CCM certification preferred.
- 5 years recent relevant experience.
- Broad knowledge base of healthcare delivery and case management within a managed care environment.
- Comprehensive knowledge of utilization review, levels of care, and observation status.
- Awareness of healthcare reimbursement systems, including HMO, PPO, PPS, CMS, and value-based reimbursement models.
