About the Role
As a Clinical Manager RN, you will be responsible for directing the overall operation of our clinical services within the Home Care Home Based (HCHB) system. You will provide oversight and coordination of referrals to the agency with internal and external referral sources, ensuring seamless continuity of patient care in a home care setting. Your proactive and collaborative approach will be essential in maintaining a positive and productive team environment.

Responsibilities
  • Direct the overall operation of the clinical services within the HCHB system
  • Provide oversight and coordination of referrals to the agency with internal and external referral sources
  • Maintain continuity of patient care in a home care setting
  • Proactive, collaborative team member in the home care environment
  • Current, active, and unrestricted Registered Nurse (RN) licensure in the state of practice
  • Two (2) years recent practical experience as an RN
  • One (1) year experience in Home Health required
